Pre-Registration Requirements

Submit the following requirements to BPLO office Window 2 & 3

  • DTI/SEC/CDA
  • OCCUPANCY PERMIT OR ANNUAL INSPECTION CERTIFICATION
  • BRGY. CLEARANCE
  • PICTURE OF THE ESTABLISHMENT
Post-Requirements

Subject for compliance of the ff. documents within thirty (30) days upon issuance of the permit.  Failure to comply will result to revocation of Business Permit.

  • MENRO CLERANCE
  • SANITARY PERMIT
  • FIRE SAFETY INSPECTION CERTIFICATE
  • ZONING CLEARANCE
  • OTHER DOCUMENTS, CLEARNCE OR PERMIT IF APPLICABLE

RENEWAL of Business Permit

Pre-Registration Requirements

Submit the following requirements to BPLO office Window 2 & 3

  • PROOF OF INCOME (e.g. LATEST INCOME TAX RETURN, AUDITED FINANCIAL STATEMENT, SWORN STATEMENT)
Post-Requirements

Subject for compliance of the ff. documents within thirty (30) days upon issuance of the permit.  Failure to comply will result to revocation of Business Permit.

  • FIRE SAFETY INSPECTION CERTIFICATE
  • SANITARY PERMIT
  • OTHER DOCUMENTS, CLEARNCE OR PERMIT IF APPLICABLE
