3. Parental Advice for Age for 21-24 years  (below 25 years old)
4. Parental Consent for age 18-20 years (below 21 years old)

5. Certificate of No Marriage from NSO Two (2) photocopy & original
6. If widowed/widower – death certificate of spouse

    For foreigner:
     Legal capacity to contract marriage
     Passport
     Birth certificate
     Divorce papers – if divorcesd
